The preliminary report into the Air India crash which killed at least 260 people has found that fuel supplies to both engines were apparently mistakenly cut off – moments after the plane took off.

According to the report, amid confusion in the cockpit one of the pilots transmitted a Mayday signal and the switches were turned back – but it was too late.
