The deaths of some babies at Countess of Chester Hospital "could have been avoided" and some attacks could have been prevented, Lady Justice Thirlwall says in her inquiry report.
The long-awaited report into the circumstances surrounding Lucy Letby’s killings of seven infants is incredibly critical of failures among hospital management, culture and executives.
