The way the Southport killer Axel Rudakubana was handled under the counter-terrorism Prevent programme has found that his repeat referrals and history of violence weren’t fully explored, according to a new review.

Security minister Dan Jarvis told MPs that "too much focus was placed on the absence of a distinct ideology" – as he accepted all 14 recommendations to improve the system.
An internal review into how Prevent deals with the "full range" of threats is also underway.
