Health secretary Wes Streeting speaks on the new report into the failings of Valdo Calocane which identified multiple issues with his mental health provision before killing three people in Nottingham last summer.
Calocane fatally stabbed 19-year-old students Barnaby Webber and Grace O’Malley-Kumar before killing 65-year-old caretaker Ian Coates.
Mr Streeting says, "the hard truth is had the NHS done its job, had there not been multiple fundamental failures, three innocent people might still be alive."
