
"Fundamental failures" and "concerted efforts" to pin the blame on Liverpool fans during the Hillsborough disaster would have resulted in gross misconduct proceedings, an investigation has found.
A new report published by the Independent Office for Police Conduct (IOPC) has found that 12 officers would have faced misconduct proceedings under current laws.
However, the IOPC says that proceedings cannot be brought because legislation at the time of the investigation prevented officers from being disciplined after they retire.
Ninety-seven Liverpool supporters died after the FA Cup semi-final on 15 April 1989.
