The Grenfell inquiry has shown there is "widespread incompetence" in the construction industry, with hundreds of other buildings with the same cladding system used by Grenfell Tower.
Inquiry member and architect, Thouria Istephan, said everyone in the construction industry needed to "play their part" to deliver change.
She became emotional as she spoke of the impact the process has had on her, saying: "It has left a mark on me as a person and as a professional which will last far beyond this inquiry."
