A long awaited report into police conduct at the Hillsborough disaster has been published.
The police watchdog, the Independent Office for Police Conduct, found that 12 police officers would face gross misconduct proceedings if they were still serving.
According to the IOPC, police were guilty of "deep complacency", "fundamental failure" and a "concerted effort" to blame fans during and after the match.
A crush on the terraces during the FA Cup semi-final at the stadium in Sheffield resulted in the death of 97 Liverpool fans on April 15 1989.
