Mistaken identity in fatal car crash

South Yorkshire Police have referred itself to the police watchdog after it emerged it had wrongly identified a victim in a fatal car crash in December.

Police initially believed 17-year-old Trevor Wynn died in the incident near Rotherham, but over two weeks later realised 18-year-old Joshua Johnson was killed.
