Pensioner killed by psychiatric patient 'failed on every level', says family

The family of a pensioner who was stabbed to death by a woman who fled an acute mental health unit have said their "nightmare never ends", and they were "failed on every level" by two police forces and one NHS trust.

A coroner ruled on Thursday that 74-year-old Roger Leadbeater was unlawfully killed, after patient Emma Borowy was permitted to go on leave, when this request would have probably been rejected if procedures had been followed.

Retired bus driver Roger was stabbed multiple times by Emma Borowy, 32, as he walked his dog Max in a Sheffield park in August 2023.

The pensioner suffered a total of 75 sharp-force injuries, his dog refused to leave his side after he died.
