More than 500 mothers and babies suffered potentially avoidable harm or died at a “toxic” hospital trust, a damning report has revealed.
The biggest maternity review in NHS history has exposed 162 deaths – including 156 children and six mothers – could have been avoided if they were given better care.
The Ockendon report, an independent inquiry into maternity services in Nottingham, has included the cases of 2,500 families over a 20-year period, and serious and deadly incidents in maternity care and found a string of "repeated failures".
Families of babies who died or were left seriously disabled due to mistakes at Nottingham University Hospitals NHS Trust (NUH) have received the final report of the independent inquiry today.
