Two young deaths at Scottish prison 'might have been avoided', inquiry finds

An inquiry has found "systemic failures" contributed to the deaths of two young people who took their own lives whilst in custody in a Young Offender’s Institution, in Polmont, Edinburgh.

21-year-old Katie Allan was found dead in her cell in June 2018. Four months later, 16-year-old William Lindsay also died in custody.

The inquiry has made 25 recommendations to try to prevent a similar tragedy.
