Babies could have been saved: the damning verdict of the Lucy Letby Inquiry. Former nurse Letby killed seven infants and tried to kill seven more.
The report describes a "complete failure to protect babies" at the Countess of Chester Hospital.
The report makes a series of recommendations including cameras on all baby cots. The conclusion: "it must never happen again".
